Chart reconciliation: tag, sign and export

CaseSync pulls a call's evidence into one timeline: CAD times, monitor data and, where your agency uses them, body-camera clips. You walk through it, tag what happened, sign, and download a draft chart for your ePCR. Nothing reaches your chart unless you confirmed it.

1. Find your call

From Paramedic Home, open My Calls. The Active tab lists your open calls. The Captured column shows which sources arrived: Cam, Mon, CAD and Chart.

Each call has a status:

  • Sources arriving. CaseSync is still waiting on CAD, monitor or body-camera data.
  • Ready to reconcile. The sources are in. Select Reconcile →.
  • Complete. The chart is handed off. Any QA findings show up under My QA Findings.

Calls where you were a secondary or observer are listed under Also on. You can view them, but the lead paramedic owns the chart.

2. Tag the moments that matter

On the Reconcile screen, play the video or move along the timeline to the moment you want. Then select Tag here, or right-click the video or the timeline.

Pick a tag type (Medication, Procedure, Observation, Vitals snapshot, Note or Custom field), fill in the fields and select Save tag.

A saved tag becomes a confirmed card right away and joins your chart export. Your tags are listed in the Chart tags panel on the right. They save as you add them, so you can select Save & continue later and come back.

3. Controlled substances need a witness

A controlled-substance card needs a co-signature from a second person who was on the call before it counts. The witness has to do it from their own sign-in. Nobody can co-sign for someone else, and you can't witness your own.

The witness finds the request on Narcotics, under Your pending CS witnesses, and selects Witness co-sign →.

4. Sign and export

When your tags are in, select Confirm & Export to ePCR. Read the summary and select Sign off & export. You're signing the chart as yourself.

Only the lead paramedic on the call can sign. If you're taking over a call, make yourself lead from the crew panel first. You also need at least one confirmed tag.

CaseSync then builds a NEMSIS XML draft. When it's ready, select Download NEMSIS XML. You get a file named nemsis-<call number>.xml. Upload it to your ePCR and finish charting there.

5. Adding something after you've signed

Add the tag on the Reconcile screen as usual. CaseSync warns you that the chart is already signed. Select Amend & re-sign… to save the tag and sign again in your name. The earlier signature stays in the history.

Then regenerate the NEMSIS export and upload the new file, so your ePCR has the addition.

Replying to your reviewer

When a reviewer leaves a note on your call, it appears in the QA ↔ paramedic thread on the call's review page. Open the call from My Calls with Review →, type your reply and select Post reply. The thread is kept. Nothing in it is deleted.
